Volunteer Application Form  (Microsoft Word Document)

  • Note 1- Please read all the instructions carefully at the beginning of the application form.  
    Application forms should be completed and sent to admin@totaifoundation.com.
  • Note 2 - Wherever possible, please send your application forms at least 3 months before your
    desired visit.
  • Note 3 - Please send a digital photograph (head & shoulders) of yourself attached to a separate e-
    mail to the e-mail address in Note 1.
  • Note 4 -  Before completing an application form we would suggest that you contact us at the
    above e-mail addresses first to inform us of your interest.
  • Note 5 - Applicants with concerns about health considerations (e.g. availability of prescription
    medicines, allergies, facilities for those who suffer from anaphilactic reactions) should contact
    the Foundation Totai in Bolivia before filling in the above application form at
    admin@totaifoundation.  Your enquiry will be responded to by our Health Centre staff and
    treated with strict confidentiality.
